What are the requirements to reinstate my lapsed education plan?
To reinstate by updating, you will be required to: 1. Fill out and sign Reinstatement form*. For plans with a Co-Planholder, the Co-Planholder is required to sign in the Reinstatement form. If the Co-Planholder did not sign the reinstatement form, it will be processed without insurance benefit. 2. For Heritage plans, sign a new applicable Masterlist of Schools (MLS). 3. Pay arrearages, penalty charges, and reinstatement fee * Form/s available at Prudentialife branches and can also be downloaded at the Forms Donwload Page of the Planholder E-Services Facility. Online Plan registration is required prior to access of the Planholder e-Services Facility.
