An experienced rating has to comply with the following requirements: 1. Accomplished Application Form (available without cost) 2. Two (2) passport-size photo (with name in print at the back) 3. Medical Certificate (in PEME form from a DOH-accredited medical clinic) 4. Certified true copy of the Seafarers Identification and Record Book (SIRB) 5.
