What are the requirements in filing a claim?
The following documents must be submitted to eStandard Insurance within 10 days for processing : If claim is due to Accidental Damage: • Proper Identifications • Bring the damaged unit • Accomplished Cell-phone Claims Report Form (CCRF) • Payment Slip/Certificate of Insurance • Any requested documentation to support the claim If claim is due to LOSS (Robbery) or Total Damage: • Proper Identifications • Payment Slip/Certificate of Insurance • Original or Duly Certified true Copy of police report or excerpt from the blotter (reported within 24 hours from the time of loss) • Accomplished Cell Phone Claims Report Form within 48 hours • Copy of any document necessary to support the claim (proof of ownership indicating the Serial Number of the insured cell-phone: invoice, box, or billing)
