• You must submit a Supplemental Application for International Students (SAIS form/Canada) (PDF) or (DOC) at the time of application. • You must be admitted to DMU as a full-time, on-campus student (part-time and on-line students are not eligible). • Students in clinical programs (DO, DPM, PA, or DPT) and accompanying dependents must carry DMU’s student health insurance plan.
