What are the requirements for obtaining a waiver of authorization to access identifiable information for research?
An Authorization (i.e. written agreement obtained from a potential subject to allow a covered entity to disclose his/her protected health information, PHI) must normally be obtained by a covered entity before the information can be disclosed to someone outside that entity. An exception to this rule is when the information is first “de-identified”.
