-Register and provide contact information and license number -Complete the cover sheet, course exam in INK with a minimum score of 70% -Complete and survey -Submit cover sheet, exam and survey to: Mail to: 6308 Circle Oak Dr, Bulverde , TX 78163 Email to: OTCourse@gvtc.com Fax to: 830.438.4573 Online course certificates are emailed to you as soon as you pass the exam. You should retain the certificate as documentation that you completed the course. 11. How quickly do you grade the exams? FAST.
