Family physicians must be certified by the American Board of Family Medicine (ABFM) and must be Diplomates in good standing in order to apply and take any CAQ examination. Diplomates must hold a current, valid and unrestricted medical license, and must qualify by satisfactory completion of an ACGME accredited fellowship training program in Sports Medicine. Also required are the formal online application, appropriate exam fee and successful performance on the half-day computer based examination.
