Applicants seeking Initial Licensure must take the 2 hour Prevention of Medical Errors course prior to licensure. Send a copy of your Certificate of Course completion to the Board with your other application material.

Applicants seeking Initial Licensure must take the two (2) hour Prevention of Medical Errors course prior to licensure. Send a copy of your Certificate of Course completion to the Board with your other application materials. Once you are licensed, e-mail us your name and license number and we will add it to your Account.
