A. The requirements to be approved for the P-Card service providers are that the Service Provider must fill out a W-9 and have on file as corporation (3) or Not For Profit (4). If the service provider is a sole proprietor or a partnership they will be denied as a service provider or if they do not have a W-9 on file with the State Comptrollers Office (OSC).
