By initiative definition, physicians should document that all of the following were used to establish central venous access: cap, sterile gown, sterile gloves, sterile drapes, hand hygiene and 2% chlorhexidene. CMS recognizes that certain “emergency situations” exist that may preclude the use of some or all of the above techniques and processes. In that case, you would still be able to report the quality code but would append the 1P modifier to indicate the measure was not met due to a medical condition (i.e. emergency).
