What are the registration procedures and patient guidelines?
If you are coming to a United Shockwave Therapies Fixed Site (Park Ridge, La Grange, Hinsdale, or Tinley Park, Illinois), you will register at our front desk. The area is typically open from 6:15 a.m. to 7:00 p.m. Monday through Friday and 7:00 a.m. to 3:00 p.m. Saturdays in some locations. Hours may vary by location. Be sure to bring your: Driver’s License Insurance card List of current medications and allergies Power of Attorney documents, if applicable Medication Guidelines All medications taken while in the facility are prescribed by your doctor and administered by a nurse. Patients are typically not permitted to administer their own drugs or keep personal medications on their persons. Unless otherwise instructed, please leave all medications at home. Cell Phone Policy For the safety and comfort of our patients, cell and digital phones must be turned off upon entering the facility. Pay phones are conveniently located nearby.
