What are the reasons for occurrence of newly hatched larval head inside the hatched egg shell and how to avoid this?

When loose eggs are incubated and brushed in crowded conditions, the larvae hatched from the lower side of the eggs enter the empty egg shells by crawling and later they can not come out of the shells due to the presence of bristles on the larval body. This can be avoided by spreading the eggs in a single layer during incubation, black boxing and during exposure to light for brushing.
