To request SLA, the Soldier should submit a memorandum or DA Form 4187 answering the eleven questions contained in paragraph 3-3 of AR 600-8-10, deployment orders and current LES through command channels to the MACOM G1 who will submit to USAREUR G1 (ATTN: PSSD) for final processing.
