What are the precertification requirements?
This differs with each insurance company. For the most part, you will be asked to see several specialists for clearance. Their consultation letters, along with a letter of medical necessity from your Primary Care Physician, a diet and exercise history, and a copy of your most recent physical will usually suffice. Some insurance companies; however, have more stringent requirements. This will be explained to you in detail at your initial consultation, or you can call your insurance company to inquire as to what their requirements are. Remember, some insurance companies DO NOT COVER bariatric surgery. Please contact your carrier prior to your initial consultation to verify benefits.
