High End Radiology procedures (CT Scan, PET Scan and MRI) require preauthorization. Preauthorization must be obtained by calling our underwriter, Coventry at 1 (800) 593-2354. This is a safety measure to assure that members are not exposed unnecessarily to dangerous amounts of radiation and also to assure that these expensive procedures are medically necessary and appropriate to treat the patient’s condition. Preauthorization is not required for services rendered outside of the United States. However, the Plan will review all services to establish medical necessity. We may request medical records in order to determine medical necessity. Review the current FSBP Brochure [Sections 3 and 5(a)] for further information.
