What are the preauthorization requirements for chemotherapy and radiation therapy?
Chemotherapy and radiation therapy require preauthorization. Preauthorization must be obtained by calling our underwriter, Coventry at 1 (800) 593-2354. The Plan has a cancer management program through ITA Partners (ITA), at no additional cost to the member. It is imperative that a patient is offered the program as soon as possible after a diagnosis is made. Requiring preauthorization allows ITA to become involved at the beginning of treatment. This assures the treatment is safe and effective. In addition, ITA’s oncology nurses are available to members 24/7 to answer questions about treatment. Preauthorization is not required for services rendered outside of the United States. However, the Plan will review all services to establish medical necessity. We may request medical records in order to determine medical necessity. Review the current FSBP Brochure [Sections 3 and 5(a)] for further information.
