Both surgeons’ names need to be on Form A. The surgeon randomizing the patient is ultimately responsible for the patient and must be present for the TSC injection and the surgery. The surgeon being trained must perform the sentinel node procedure and dictate the operative note. The protocol Form SP is used and signed by both surgeons. A copy of the form is submitted to UVM as a training case, while the original is sent to NSABP as a protocol case.
