What are the plans for migrating legacy coded data?

The plans for what data is to be/can be migrated to new systems is at the discretion of the local communities migrating in consultation with their LSP. From the terminology perspective there is a national Read to SNOMED CT map provided by the NHS Terminology Service. If there are other national code sets that need to be mapped to SNOMED CT to aid migration then these may be provided by the NHS Terminology Service.
