Your Sharps+Pak® kit was shipped with individual tracking documents. You must complete the generator section of the form (if not filled out), sign it, keep the generator copy and seal the remaining copies back in the shipping envelope. When your waste has been destroyed, you will receive a final copy of the tracking document, showing that it was properly destroyed.
