For babies born on or after May 1, 2010, leftover blood spots will be stored but NOT used for research unless a signed parental consent form is on file with the state laboratory. You may ask for the specimen to be destroyed using the “Directive to Destroy” form above. If you were not offered a chance to consent for future research using your baby’s blood spots and would like to provide your permission now, please read the consent booklet first and then complete the consent form below. If you were offered the chance and did not consent when your baby was born but have changed your mind, you can also still fill out the consent form now.
