Request for Extension of Incomplete Grade Form located in the Office of the Registrar, Joyal Administration Building, Room 106. The Request for Extension of Incomplete Grade form must be signed by both the student and the instructor and submitted to the Office of the Registrar prior to the last day of the semester in which the grade must be made up.
