What Are the Optimal Therapeutic Ranges?
The optimal therapeutic range for laboratory evaluation of oral anticoagulant therapy is now established. The intensity of anticoagulation recommended for many indications has been reduced because of the results of randomized studies. The guidelines proposed in 1989 by an advisory group of the American College of Chest Physicians have recently undergone modification. Two levels of intensity are recommended: (1) a less intense effect with a lower INR (2.0-3.0), and (2) a more intense effect with a higher INR (2.5-3.5).
