What are the new co-sponsor responsibilities for MTI classes?
All MTI classes will use standardized daily sign-in sheets, registration forms, evaluation forms, and certificates of attendance. The co-sponsors will be responsible for returning completed forms to MCC and generating certificates. See checklist and instructions for MTI forms. The use of the new seminar evaluation forms is effective immediately for all CET seminars. There are two evaluations forms, with the same content. For MTI courses, the evaluation form has the MTI logo. The evaluation form for other CET seminars only has the MIOSHA logo.
