The application package is lengthy and contains two main parts. One being PAE’s forms, the other the State Department’s. The applicant does not fill out this form. It is intended for a Spouse or Cohabitant to sign. Failing to list contacts with full name, address, and phone number. Failing to sign and date all forms that require signatures. It is important to have signatures and dates on all forms that require them. A thorough “final run” is suggested to avoid any missing signatures.
