The monitors are looking to see that the provider has met once a month, either face to face or over the phone, with an individual specialist-level credentialed provider or group, of which at least one member is a specialist-level credentialed provider. The Ongoing Professional Development Form needs to be completely filled out with the date of the meeting or phone call, location, and signature of the credentialed peer. Phone call meetings are required to have the signature of the specialist-level credentialed provider.
