What are the medical/vision questions and other certifications?
Medical and Vision Questions: To renew your driver license on-line, you must answer several medical and vision questions: • Medical Conditions – You must answer questions about whether you have, or were treated for, conditions that include: a hearing impairment, a heart ailment, a lost use of a limb or eye, a convulsive disorder or any condition which causes unconsciousness. • Vision Exam – An eye-care professional must complete form MV-619 or MV-619R (Eye Test Report) for you. You must enter information from the form to complete your on-line driver license renewal. If the form MV-619 has a print date of 9/03 or earlier, the form is valid for 6 months after the date of the eye test. If the form has a print date of 3/04 or later, the eye-care professional marks a box on the form to indicate if the form is valid for 6 months or for 12 months after the date of the eye test.
