For Kindergarten through 5th grade the State of Oklahoma requires the following: 5 DTP shots, 4 Polio shots, 2 MMR shots, 3 HepB shots, 2 HepA shots and one Varicella shot. For further immunization information info please call OK State Dept. of Health at 405-271-4073. Student’s shots should be up-to-date prior to enrollment.
