At a minimum, the fellowship requests the following information from applicants: Completed AMSSM Application 3 letters of recommendation (1 from your residency’s program director) Curriculum vitae Personal statement Copy of USMLE/COMPLEX transcript Copy of medical school diploma & transcript Certificate or other validation of all previous training Copy of current state medical licensure Please refer to the Application Information section of this website for more information.
