Gaming service provider Certification Application and Disclosure Information Form – $1,500 Gaming service provider Certification Form – Private Holding Company – No fee Pennsylvania Personal History Disclosure Form – $500 Gaming Employee Application and Disclosure Information Form – $350 Non-Gaming Employee Registration Form – $60 The gaming service provider must also pay a $4,000 certification fee prior to the issuance of its certification.
