What are the eligibility requirements for medication assistance?
To qualify for assistance you must be a U.S. citizen or legal resident and meet the required income guidelines (based on 200 percent below the Federal Poverty Level). Proof of income is also required such as a Federal tax form or pay stubs. A current prescription from your provider has to be included also.
