Annexure H duly signed by the parents Birth Certificate issued by authorities concerned under RBD Act Parents passports Address proof if it differs from parents passports In case of either parents consent not available, affidavit as per Annexure G If one parent is abroad, Sworn Affidavit duly attested by Indian Mission along with an affidavit from the parent residing in India Three latest passport-sized color photographs with a clear background Affidavit as per Annexure C to be submitted with the application for a passport of a minor child by either parent (who are separated but not formally divorced) or by a single parent of the child born out of wedlock In case the parent applying is a divorcee, furnish Court Order giving custody of the child.
