The difference is that water is injected into the cylinder at the end of the exhaust stroke. The valves close and the heat of the engine turns this water into steam. As the steam expands there is an additional power stroke. The idea is that it gets an extra power stroke for next to nothing. An additional benefit is that the water and steam give additional cooling to the engine.
