• Grade 1 – Comedones with less than 30% present. • Grade 2 – Comedones and some pustules. • Grade 3 – Comedones, inflamed pustules, and an occasional inflamed cyst. • Grade 4 – Large pustules, cysts, inflamed nodules and scarring. This is called acne conglobata.
