However, in order for a child to be enrolled in a Head Start program site, his/her family must meet income eligibility guidelines. Q: How do I enroll my child in Head Start? Fill out the Three-Choice form and indicate the selection of Head Start. The principal at your site will forward all Three-Choice applications indicating an interest in Head Start to the Head Start Service Center, located at 5225 W. Vliet Street, (414) 777-7850. A Head Start staff member will contact you and will help you with the registration process. Children cannot be enrolled in Head Start without completing all necessary forms which include income verification.
