NJ DIRECT 15 is a PPO plan administered by Horizon Blue Cross Blue Shield of New Jersey. The plan is similar to the current NJ PLUS plan in that it covers treatment from both preferred and non-preferred providers. NJ DIRECT 15 offers a national network of providers, does not require members to choose a primary care physician, and does not require referrals. These features are not available under NJ PLUS. The copayment for NJ DIRECT 15 is $15 for services received from preferred providers. NJ DIRECT 15 allows for treatment of injury or illness by non-preferred providers with a $100 deductible and 70% coinsurance for most services.
