Answer If the account is in your name, you are responsible for arranging for service disconnection and paying the closing bill. We will need your name, account number, the date your service is to be disconnected, and your forwarding address and telephone number. We request at least two (2) working days advance notice to ensure services are disconnected on the date specified.
