Doctor’s Prescription for Therapy Insurance Card Photo ID or Parent Photo ID for a Minor Legal Guardian Must Be Present for a Minor Under Age 18 Loose clothing that Exposes Body Part Being Examined (i.e., shorts, tank top, loose t-shirt) • What happens if the weather is inclement? Our patient’s safety is our NUMBER ONE concern. It is PTI’s policy to continue operations despite inclement weather unless the weather threatens to make patient or employee transportation to or from the clinic dangerous or impossible. If you do not feel comfortable driving in the inclement weather we will honor your cancellation without charge. We will do our best to contact you if your appointment will be impacted by delayed openings, early closures or closure. Updates on any closures will be posted on our answering service as soon as they are made. If the weather looks dangerous, or poor weather is forecasted please call 508.721.0000 ext. 5.
