Additional information is often requested on the distribution and collection of consent forms and/or research instruments and on how the data, including the consent form, will be stored and who will have access to the data. The consent form may be missing items or may not differentiate between confidentiality and anonymity. There may be spelling errors on the consent form or research instruments.
