The morphine primarily is coded one of two ways: 1. J3490 (unclassified drugs) is recommended for infusion pump refills by some carriers. A 2003 article by Cigna’s Part B carrier states that J2275 (injection, preservative free morphine sulfate per 10 mg) is not intended for the quantities used in infusion pump refills and therefore, J3490 is the recommended code. 2. J2275 (injection, morphine sulfate, preservative free solution per 10 mg) is the other code that has been used.
