This is much more likely to have been caused by an inconsistent pill schedule, which makes the pill much less effective. Also, unless her ovulation schedule was abnormal, she didn’t get pregnant while on the sugar pill, but most likely because she delayed starting a new packet of birth control for an extra day or so after the sugar pill, making it possible for her to ovulate and conceive while on her pills.
