• Providers who participate in the settlement and file adjusted reimbursement claims pursuant to the terms of the Protocol will share in additional MA reimbursement payments of up to $16,000,000 over the next 2 years to pay for uncompensated care. What do I need to do? • You need to submit two documents to the Department: (1) a Provider PEME Claim form, and (2) a Provider Declaration. The Provider PEME Claim form must be submitted to the Department no later than Friday, June 25, 2010.
