The outpatient benefits are flexible and are designed by the Employer. Typical coverage includes benefits not covered by the comprehensive major medical plan and can include: office visits, outpatient x-ray & lab, physical therapy, mental & nervous providers, durable medical equipment, acupuncture, chiropractic charges and other professional services. The Employer also has options with how the major medical, hospital in-patient co-insurance and deductibles are funded.
