If the individuals have not had a level 2 (FDLE and FBI screening) background screen conducted, then those individuals must submit a fingerprint card to the Background Screening Unit at the time an application or a change in personnel is sent to the Home Care Unit. If the individuals have had a background screen within the previous 5 years to meet any provider or professional licensure requirements of the Department of Health, the Agency for Persons with Disabilities or the Department of Children and Family Services satisfies the requirements of subsection 408. 809, F.S. provided that the proof is accompanied, under penalty of perjury, by an Affidavit of Compliance with Background Screening AHCA Form 3100-0008, November 2006.
