Students must provide three (3) types of acceptable identification from the list below to prove their identity when applying for a National Criminal Record Check, one of which must be the Student Identity Card. Identification must be originals, not be expired and should also include at least one type of photographic ID and identification that contains a signature and date of birth. Applicants must record in the space provided in the application form the identification type (eg Medicare card) and number (if applicable).
