What are the 8 pathways or models of care?
This local work forms part of a wider national process, known as Our NHS, our future, to develop the future vision for the NHS, being led by Health Minister and practising surgeon Lord Darzi. The national report was published on 30 June 2008, and provides a framework within which our work in Shropshire, Telford and Wrekin is being developed. The eight pathways, also known as models of care are the clinical areas that we are focusing on. These areas of work have been set as part of the national Darzi work. Below is a list of the pathways with a brief summary of the key priorities and areas of focus from our plans so far.
