Before you can get an upgraded account, you will need to: • be a Veteran enrolled at a VA health care facility • register as a VA Patient in your My HealtheVet account (in the Profiles section under the Personal Information tab) • view the My HealtheVet video (available online or at your local VA facility) • read and sign a printed copy of VA Form, 10-5345a-MHV (available online or at your local VA facility). Do not forget to take your signed form to your local VA facility and government identification to complete the IPA process. You can upgrade your account at your VA Medical Center or Community Based Outpatient Clinic (CBOC).
