Box 02-5450, Miami, FL. 33102-5450. Be sure to include the yellow citation along with the driver license number and defendants name to ensure proper payment application. DO NOT SEND CASH IN THE MAIL. If you are paying by using your Visa or MasterCard, you must indicate which credit card you are using, enter the credit card number, the expiration date, print your name as it appears on the credit card and sign your name. Please be aware, should the clerk receive your payment more than 30 days from the issue date you may be subject to a driver license suspension by the State of Florida.
