If you are pregnant or wish to take Paternity leave following the birth of your child, please download and complete the appropriate form below and send it to your local Unitemps office. We will then contact you to confirm your entitlements for leave and payment of SSP/SSP where appropriate. We can also arrange for a risk assessment to be carried out for your current assignment.
