For Schedule of Benefits I, maximum out-of-pocket (in network) is $5,000/person or $7,500/person (out of network). Effective July 1, 2006, there is a cap limit of $10,000 aggregate per family in network and $15,000 aggregate per family out of network. For Schedule of Benefits IV, maximum out-of pocket is $7,500/person. Effective July 1, 2006, there is a cap limit of $15,000 aggregate per family.
