Like the others have said demonstration sports are included as a trial to see whether they work out and are popular enough to include long-term and while the winning participants are awarded gold, silver and bronze medals those medals don’t count towards a nations final medal tally. They are chosen, I guess, through the sports lobbying the IOC for inclusion and the IOC deciding which ones might be popular and worth trialling.
